Accessing and Requesting Your Health Information
The Health Information Management (HIM) Department at Ellenville Regional Hospital is committed to protecting the privacy, accuracy, and security of your medical information, in full compliance with state and federal regulations, including HIPAA.
We offer several convenient ways for you to access or request copies of your medical records. Whether you prefer to receive your records electronically or pick them up in person, our team is here to assist you throughout the process.
To request your records, you will need to complete an Authorization for Release of Protected Health Information (PHI) form. You may download the form in your preferred language below:
Once completed, the form can be submitted by mail, fax, or email. For assistance or additional instructions, please contact our department using the information provided below.
Medical record requests are generally processed within 7 to 10 business days. You have two options for receiving your records:
1. Electronic Delivery
We recommend receiving your records electronically, as it is the fastest and most convenient option. Once processed, your documents will be sent securely via encrypted email.
2. In-Person Pick-Up (By Appointment Only)
If you prefer to pick up your records in person, please note that appointments are required. Once your records are ready, you will be contacted to schedule a convenient pick-up time.
Please bring a valid photo ID when you arrive. Enter through the main entrance of Ellenville Regional Hospital, where a HIM team member will meet you in the main Registration area.
There is no charge for obtaining copies of your medical records.
