The Ellenville Regional Rural Health Network (ERRHN) addresses social determinants of health by providing dedicated support through Community Health Workers. Social determinants of health are the non-medical factors that influence health outcomes—such as the conditions in which people are born, live, learn, work, play, worship, and age—that impact a wide range of health, functioning, and quality-of-life outcomes.
Our Community Health Workers work directly with community members to identify and navigate barriers to well-being, including food insecurity, lack of transportation, housing instability, and limited access to healthcare. By connecting individuals with essential resources and support services, we empower them to overcome these challenges.
Through this approach, ERRHN promotes health equity, enhances overall health outcomes, and helps build a more resilient and supportive community for all residents.
