New Hospital Management System (HMS) Revolutionizes registration

In the past, when you registered, even if you had been here before, we needed to ask you a lot of questions, and get your history, including prior illnesses, not always convenient in an emergency.

Registration

ERH now has a state-of-the-art electronic health record system designed to save time in most instances.

For many outpatient services such as Diagnostic Imaging and Ambulatory Surgery, we pre-register patients ahead of time so your time spent in Registration on the day of your procedure is minimal.

In the Emergency Department and Acute Inpatient areas, we may ask you questions you have answered before, even if you are in the system, as a safety precaution.

Documents to Bring With You

The kinds of details you will need to supply are:

  • Phone number
  • Address
  • Emergency contact
  • Employer
  • Primary care provider
  • Insurance information
  • Proper identification
  • Prescription for any tests you are having

If you need financial assistance

When you are registering is a great time to ask about financial assistance. We have the means to help you and, if you wish, you can download and complete the necessary forms and bring them with you. Click here for more information.

Discharge

Most of the time, the system automatically discharges you, but occasionally, you will be asked to sign discharge forms.

Medical Records

Please call us at 845-647-6400 ext. 232. Please be aware that HIPAA privacy regulations require us to follow very specific rules in order to give copies of your medical records to you.

Releasing your own records or for minor children

We can release them as long as you bring photo ID and sign a release form. Click here for a PDF copy of this form.

Releasing another person’s records

We cannot release records to a spouse, adult children, or parents without the patient’s written permission or by producing a medical proxy or power of attorney form.
